唐山（甄子丹 饰）留洋学得医术后归国，与助手刘国邦在上海闸北的贫民区开设西医诊所。当地斧头帮恶霸于老七（于荣光 饰）为了得到一块地皮，经常带人殴打村民，唐山与刘国邦出手制止行凶，又义务诊治受伤村民。为制止于老七，唐山拜访了那块地的主人——自己在英国时的老师西蒙，以为西蒙工作为代价，唐山保住了村民的土地。于老七的妹妹于小倩（朱茵 饰）多年来患有喉疾，不能说话，笃信狐仙的于老七以为求仙可令妹妹痊愈，排斥西医治疗，小倩机缘巧合结识唐山，后者的诚意打动了小倩，令其接受了手术。与此同时，贫民窟中的孩子们陆续失踪，唐山发现此事的幕后主使是斧头帮和西蒙，一场恶战在所难免。©豆瓣Tangsan is a doctor living in the poverty stricken area of Shanghai. All the poor people come to him for help because of his kindness. But soon, the Axe Gang enters the picture, and they're ploting to destroy the town so they can build their own casino. Tangsan naturally gets involved, and starts an ongoing rivalry with the gang. But the gang leader's sister meets Tangsan one day, and they begin to have feelings for one another which complicates matters even more.
